In order to make your first visit smooth, we have provided a list below of those things to remember before you come for your first appointment.

You will also find our new patient forms below. Please print them off and fill them out prior to your appointment. This will shorten your wait time when you come in.

• Please always bring a copy of your insurance cards.
• Remember to bring a list of any medications you are taking.
• Make sure that you have contacted your primary care physician for a referral.
• Contact your insurance provider to confirm that vision care is covered.
• Always bring a photo ID with you.
